I'm no anatomy expert however I assume they're talking about breast tissue vs body fat.
I do know that in Men there is a difference between gynecomastia (enlarged glandular breast tissue, which requires surgery to remove) and psudeogynecomastia (which occurs in obese individuals and is not due to enlargement of breast tissue, it's simply excess body fat which looks like breasts).
Some Women are busty even when slim and lean. Others have lucky fat patterning and gain weight in all the right places. The latter group are prone to major fluctuations in size due to weight gain/loss.

in the first picture she is obviously wearing a bra, you can see it under her dress. in the third picture, she is pictured from before she made under the skin. and before she took her breast implants out. and just because her breasts are smaller and she lost weight doesn't mean for sure that her boobs are natural. she could've lost weight and took her breast implants out. if you watch the scene where she's looking at herself in the mirror in under the skin, you'll see a woman who has small, A cup boobs, they are completely flat.
