Salvatore Changed His Name?

In the director's cut, Elena tells Salvatore "you shouldn't have gone and changed your name. You should have kept your own." Throughout the movie, from when he was a boy to when he became a famous director, he was named "Salvatore Di Vita," but with the nickname, "Toto." What did Elena mean by him having changed his name?

I totally forgot that I posted this question. I rewatched the new, longer version last night...and again got hooked with what Elena said and wondered why she said that! In an earlier scene, the one right before Salvatore sees Elena's daughter from the window of a bar, a fan approaches him as asks, "Excuse me, are you Dr. Lenera?"
