The 'north' is anything north of Rome, but in practical terms, the north is really the farthest north-cities like Genoa, Turin, and of course Milan... There is the resentment and jealousy from uneducated southern Italians that northerners' wealth and prosperity is due to luck... Naples is DEFINITELY in the poor south, so the totally ironic comedy of the line is that even dirt poor Neopolitans like Ciccio are considered 'northeners' when in fact he's considered a southern Italian...
