i heard an answer somewhere, but can't remember. if i do i will repost. i think it had something to do with his safe cracking. maybe, expert use of stethascope? just a guess

because hes surgical. he does flawless precision work planned to perfection.

I assumed because his surname is McCoy. But then the character was called Doc in the original as well and Star Trek was less of a cultural touchstone when that came out.
