Did this character survive? I've had this movie for years, seen it more times than I can remember, but I can never tell if this character survives or not.... Sure looks perilous towards the end, but I can't be sure.

Yeah I noticed that too, that we didn't get closure on it. But I'm pretty sure he dies. Given how much worse Rene Russo gets after checking on him, before getting the cure, I can't imagine Casey could have also progressed that much further without dying. I think he dies very soon after that conversation with Rene Russo.
