Angier's wife
For the trick in which Angier's wife died, she would usually slip out of the knot and then open the box, right? Well why didn't Cutter open the trick lock and pull her out of the tank instead of beating the glass with an axe? Clearly the axe was of no help and they did not take necessary precautions if it took so many hits to break the damn glass.
