Are they first cousins? Who was John's real father related too? Was he the mad king's brother?

Dany is Jon's Aunt and Sansa, Bran and Arya are his cousins.

His birth father was was her big brother and his birth mother was his adopted Dad's sister.

Incest is perfectly fine between consenting adults. However, there is a hightened risk to genetic defects in offspring.

The Targaryens had madness in theirs, occasionaly.
