In the syndicated version, 2 people have won. In the primetime version, I think 2 or 3.

I've seen quite a few people get 25k and then not go for the bonus. And in fact, people almost never go for the bonus. Even people who have only had 1000 or 2500 or some tiny amount don't go for it. Which is ridiculous.

Yes, I did see it once. I guess in order to say that you have to get all the questions right, plus win the bonus question.

That was Kathy Cox, Georgia's Superintendent of Schools at the time, who announced she was donating her winnings to schools for the deaf and blind. However, she declared bankruptcy when her husband's business failed, and in the ensuing legal mess, the schools got half the money and the other half went to creditors.

She won the million by answering that the longest-reigning British monarch was Queen Victoria. (That's still true as I write this, but won't be as of September 2015 if Elizabeth II is still on the throne.)
