Jason's last name was Cancam. I'm sure that was his stepfather, Kofi's last name. Jason referred to Kofi as his dad so Kofi probably adopted Jason when he married Jason's mother and because Jason's father was dead. The movie never mentions what Kofi's last name is, but since Cancam wasn't Jason's father Luke's last name nor is it his mother's maiden name, then it would have to be Kofi's. Avery probably already knows Kofi's last name because he and Romina were most likely questioned by police about Luke so that information would be included in the police reports. Avery also went to Kofi and Romina's house that night so those other cops could have told him off screen what their names are. Avery also knows that Luke's son's name is Jason and that he is the same age as his own son. So I assume that when Avery comes to the police station when his son and a friend are arrested, its not hard for him to figure out who Jason is by putting all of this information together. Also, seeing Jason on the camera monitor at the police station, Avery may have also noticed a resemblance to Luke.

I think that even if the last name had not come up in the news or paperwork from the original case, then maybe Avery had kept an eye on the kid, like at least just to learn his name and maybe appearance.
The point though, Kofi or Coffee Can is a very odd name.

Kofi is a very very common name in Ghana. Every male baby born in the South of Ghana on a Friday is called Kofi + receives a christian name when he survived the first seven days.
