One body.

"Gabriel" only ever existed as two stubby hands, some brain matter, and a fucked up face (which is more or less brain matter I think). The surgery at the beginning removed most of that from Madison's body. The only thing left is some brain tissue in Madison's head.

After the head injury, Gabriel found that it was able to control Madison's body and mind.

Mostly just one. In flashback they removed most of the 'cancer' ... a residual portion they couldn't remove because it would kill her, so they tucked that back inside her skull and closed it up.
