When he says "i think you've been holding out on me. I bet if I hit the right spot I can make you scream" Just a reaaaaally *beep* sexual reference?

He was obviously a psycho but I think he meant that if he stuck the knife in the right spot he could make her vocal chords work for her to scream before dying.

While that's what he means literally, I think that the line intentionally has a sexual sound to it. Not to be crude about it, but causing pain and terror is how he "gets off". In my opinion the line is creepy because it has a double meaning--most people talking about "hitting the right spot" and "making a girl scream" would be describing a successful sexual encounter. This guy is talking about stabbing a mute woman in the neck.
