Indeed the montage answers most questions--- but I must have missed something.. Who/When/How Killed Miranda? Did this also happen during the kerfluffle in the boat?

-- And I see two explanations here -- interested who responds.. the entire beginning -- was that REAL and it happened many years ago? ( So when they get to the campsite and get out of the RV it's really many years later) .. OR is the whole beginning NOT real and part of their delusions?

The beginning is exactly that vcr record that is playing at the final. It was not a delusion, it was only the record.

When then arrive at the campsite, only the couple and the dog appears at the windows, and for a moment they also don't mention the girl (which was unfortunately dead 7 years ago).

Miranda just drowned. Just that.
