Do you arrange movie titles starting with an "A" as A?


Or omitted the "a" and arrange according to the first letter of the second word?

I always put "the" at the end so that my movie collections sorted by the second word instead (e.g., LEGO Movie, The) would be in L and not T, but keep A in front (e.g., A Separation) so that it would still be in A and not S.

Wikipedia uses BOTH (so everything duplicates) in their list of films.

It's better with an A.
For example, A Clockwork Orange certainly sound better than Clockwork Orange.

I think so too. While "Last King of Scotland, The" is okay "Clockwork Orange, A" looks awkward.

I arrange by genre only

But genre is fuzzy. How to arrange genre defiying movies? Like the recent Korean film Parasite for example... is it comedy? Or thriller? Or anything else?

I didn't buy the recent Korean film Parasite, so....

Every film has its MAIN genre and then come a few sub-genres

That's OK. I've never had any problems or confusion with my personal arrangement by genres.
