there is a supply chain of information and responsibility that is supposed to be adhered to scrupulously when a firearm is involved in any scene. one single disruption of that chain can have fatal consequences. the only thing to do now is to pinpoint exactly where the system broke down. it is likely with one person, although we may find there are contributing factors as well. in any event, it's an understatement to say it should've never happened. it's human error 100%. and totally preventable. sadly, this will be the event that forces change in the industry.

IMO, "The Entire History of You" might be the best single episode of television ever.
Some other good ones, depending on your mood are "White Christmas" and "White Bear" [serious] as well as "Nose Dive" and "Hang the DJ" (funny).
People love the 80s assisted suicide one too.
