Whenever my health insurance is through a COBRA plan I usually pay by check since they want to charge a $20 service fee to pay it online.

Don't you just love it?!!! They charge you a fee for getting their money instantly.

I can understand it to some extent because of the fee credit card companies charge them and since it is a necessary item and not a consumer item they have no interest in giving someone an incentive to purchase. But I have mostly encountered this just with COBRA. I have private insurance now and although they don’t accept credit cards you can still pay online without fees if you give them your bank checking info. It seems to me that in COBRA plans the company really wants to get former employees off the plan so they make it more inconvenient to pay premiums.
